Job Summary

We are seeking a Registered Nurse to join our team of dedicated healthcare professionals. As a Registered Nurse with Agrace, you will have the opportunity to work with patients and families in a variety of settings, providing end-of-life care and support.

Key Responsibilities
  • Provide patient and family-centered care, assessing physical, emotional, and spiritual needs
  • Develop and implement individualized plans of care, working collaboratively with the interdisciplinary team
  • Conduct patient visits, providing interventions and treatments as needed
  • Document patient information and outcomes in electronic health records
  • Participate in meetings and training sessions to stay up-to-date on best practices and industry developments
Requirements
  • ADN or BSN degree in nursing
  • Current, unrestricted Registered Nursing License in the state of employment
  • One year of nursing experience, preferably in hospice or end-of-life care
  • Valid driver's license and access to a reliable vehicle
  • Ability to work effectively and cooperatively with others
  • Ability to communicate effectively verbally and in writing
