Job Summary: We are seeking a compassionate and skilled Registered Nurse to join our PruittHealth Hospice team as a Hospice Care Specialist. As a key member of our interdisciplinary team, you will provide high-quality patient care, coordinate services, and ensure a seamless transition for patients and families into our hospice program.

Responsibilities:

  • Screen referrals for appropriateness and perform admissions to facilitate the transition of patients and families into the hospice program.
  • Coordinate the interdisciplinary care team for assigned patients and implement the plan of care.
  • Provide primary nursing care for hospice patients, regardless of the setting.
  • Assume full responsibility for professional management of assigned hospice patients' care related to their terminal illness.
  • Teach primary caregivers skills needed to care for patients in their residence.
  • Maintain clinical records to meet federal, state, and agency regulations and guidelines.
  • Supervise hospice aides and certified nurse aides, making supervisory visits at least every 14 days and face-to-face visits at least monthly.
  • Participate in Performance Improvement Programs and present inservice programs, team conferences, and staff meetings.
  • Provide skilled nursing care to assigned patients/families as outlined in the plan of care.
  • Participate in the development of patient/family interdisciplinary care plans.
  • Verify patient visits and review documentation to ensure caregivers are carrying out the plan of care as written.
  • Have ultimate responsibility for developing and integrating the plan of care with the hospice team and SNF team to ensure one integrated plan of care that reflects the hospice philosophy.

Requirements:

  • A thorough understanding of Hospice Regulations (federal, state, and local government).
  • Ability to handle confidential matters.
  • Ability to work a flexible schedule.
  • Commitment to the Hospice concept and standards.
  • Participate in agency surveys and any subsequently required reports.
  • Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
  • Honor patients/residents' rights to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights, including the right to wage complaints.
  • Follow established safety regulations, to include fire protection & prevention, smoking regulations, infection control, etc.
  • Follow established safety procedures when performing tasks and/or working with equipment.
