Job Description

Spectrum Healthcare Resources is seeking a highly skilled Registered Nurse to fill a Disease Management position. This is a remote opportunity, allowing the successful candidate to work from the comfort of their own home.

The ideal candidate will have a strong background in nursing, with a focus on disease management and education. They will be responsible for performing assessments, providing education and training, and coordinating care for the dependents of our Nation's Active Duty and eligible retirees.

The successful candidate will have a strong understanding of chronic care management, with experience in a remote or hybrid environment. They will be highly organized, self-directed, and able to function in a high-volume environment without distractions.

The position requires a strong technical aptitude, with proficiency in Microsoft Office applications. The successful candidate will also have access to secure, high-speed internet.

Requirements

  • Bachelor's degree in Nursing
  • At least one active state nurse license, preferably compact state licensure
  • At least 2 years' experience working as an RNDM, preferably in a remote or hybrid environment
  • Certification in disease-management related area, such as CCP, CHC, CDE, or AE-C
