Job Title: LPN Homecare Nurse

We are seeking a skilled and compassionate LPN to join our team of healthcare professionals in providing high-quality care to pediatric and adult patients in their own homes.

About the Role:
  • Provide skilled nursing care and services to patients in accordance with prescribed orders
  • Assess and evaluate patient care, identifying areas for improvement and implementing changes as needed
  • Collaborate with healthcare team members to develop and implement individualized care plans
Requirements:
  • Current licensure as an LPN/LVN in the state of practice
  • Current TB or Chest X-Ray
  • Current BLS card
  • One year prior professional nursing experience preferred
