Nurse Case Manager

As a Nurse Care Coordinator at Ennoble Care, you will play a vital role in ensuring the delivery of high-quality care services to our patients. We are seeking an experienced LPN to join our team in East Point, GA, working closely with clinicians, staff, and patients to achieve healthcare goals and maintain open lines of communication.
About Ennoble Care:
We are a mobile primary care, palliative care, and hospice service provider serving patients across the US. Our clinicians use remote patient monitoring, behavioral health management, and chronic care management services to deliver personalized care to those with chronic conditions and limited mobility.
Key Responsibilities:
- Develop and implement individualized patient care plans using Ennoble Care's electronic medical record system
- Foster strong relationships with patients to provide care coordination, support, and manage compliance with care management programs
- Document all client communications accurately and efficiently
- Collaborate with Primary Care Clinicians to ensure comprehensive care plans are complete and accurate
- Stay informed of issues pertinent to the care plan process and communicate with Team Supervisors accordingly
- Work collaboratively with cross-organizational areas to meet the needs of Ennoble Care's patients and their families
Requirements:
- Must be a Licensed Practical Nurse (LPN) with relevant experience
- Must be comfortable with extensive phone conversations and possess excellent customer service skills
- Ability to multitask between different patients and workstreams while maintaining organization and efficiency
- Proficiency in using technology, including secure email, telephone system, and electronic medical records
- Knowledge of basic healthcare terms, conditions, roles, and principles is beneficial
Compensation: $75,000 - $95,000 per year, depending on experience
