Nurse Case Manager

Talent Software Services is seeking a skilled Registered Nurse to join our team as a Case Manager. As a key member of our healthcare team, you will be responsible for conducting intake assessments with patients and families, ensuring they feel equipped and supported, and developing individualized treatment plans that account for physical, financial, and other constraints. You will also be responsible for adjusting treatment protocols as needed, suggesting and coordinating follow-up services to promote wellness, and ensuring patients have transportation to and from their homes and healthcare facilities. Additionally, you will meet with healthcare teams to discuss patients' progress and deliberate on appropriate care, check for insurance coverage or finances, and devise innovative solutions or suggest referrals for patients who cannot afford treatment. To be successful in this role, you will need a Bachelor's degree in nursing, advanced degree in nursing is strongly advantageous, and prior experience as an RN case manager within a similar healthcare facility. You will also need a driver's license, own vehicle, and familiarity with patient databases. Strong leadership, supervision, and motivational skills are essential, as well as a commitment to promoting patients' well-being. Certifications such as BCLS are also required.
