IDD Qualified Professional Job Description

RHA Health Services is seeking a highly skilled IDD Qualified Professional to join our team in the Deep Gap Admin Unit. As a key member of our interdisciplinary service team, you will play a vital role in guiding the development of person-centered plans and overall programs for individuals with intellectual and developmental disabilities.

Key Responsibilities:

  • Coordinating IDT meetings and Individualized Service Plans (ISPs)
  • Assisting individuals to identify and communicate their requests and needs for services and supports through direct interviews
  • Supervising, integrating, and coordinating person-centered plans and monitoring progress towards personal, clinical, and functional outcomes
  • Initiating periodic reviews, investigations, modifications, and adjustments by soliciting feedback from individuals and their families
  • Reviewing assessments completed by individuals and other team members to best support their goals and outcomes
  • Supervising and writing person-centered plans based on assessments, interviews, and observations
  • Overseeing residential activities, including home appearance, leisure materials, cleanliness, and community integration to ensure individuals' health and safety
  • Visiting vocational centers, residential homes, and other areas to assess person-centered plan implementation
  • Reporting to the Administrator and collaborating with colleagues to maintain communication, provide feedback, and improve employee performance
  • Monitoring activities to ensure quality assurance and state survey improvements
  • Serving as a link between individuals, their families, and legal guardians to gain approval for services provided

Requirements:

  • Bachelor's degree in Social Work, Social Services, Human Services, or Special Education required; Master's degree a plus
  • Minimum 2 years of experience working with individuals with intellectual and/or developmental disabilities required; 1 year of experience if paired with a Master's degree
  • Working knowledge of applicable regulations and laws related to our service offerings
  • Valid driver's license, auto insurance, and reliable transportation
  • Supervisory/management experience and experience writing person-centered plans preferred
  • Must be at least 18 years old
