Unison Behavioral Health Group is seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in connecting children and youth with behavioral or developmental disabilities to necessary care and services.
Key Responsibilities- Develop and implement individualized care plans in collaboration with children, youth, and their families.
- Conduct thorough assessments to identify needs and develop strategies for service coordination.
- Build and maintain relationships with community partners, healthcare providers, and other stakeholders to ensure seamless service delivery.
- Provide education and support to families on available resources, services, and benefits.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
- Bachelor's degree in a related field (e.g., social work, psychology, healthcare administration) or equivalent experience.
- Minimum 2 years of experience in a related field, preferably in children's behavioral health or a related public sector human services field.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
