Supportive Services Counselor

**Job Summary:**
We are seeking a highly motivated and compassionate Supportive Services Counselor to join our team at Coarc. As a Supportive Services Counselor, you will play a vital role in empowering individuals with disabilities to achieve their goals and live fulfilling lives.
Key Responsibilities:
- Develop and maintain effective working relationships with individuals being served and their family members, ensuring confidentiality and reporting any unusual circumstances as needed.
- Provide supports in activities of daily living, community awareness, financial assistance, emergency procedures, and food shopping, among others, tailored to promote personal growth, skill acquisition, and community inclusion.
- Establish a safe and healthy environment while working with individuals in their homes, during travel, or at community events, and respond to behavioral needs as per written Monitoring Plans.
- Assist individuals in developing systems for maintaining cleanliness, personal hygiene, and laundry, and conduct monthly checks for cleanliness and safety in Supportive IRA settings.
- Provide education and training on safety in the community and within the home, and ensure compliance with safety procedures and regulations in Supportive IRA settings.
- Transport individuals to group and individual events, medical appointments, and other scheduled activities, utilizing safe driving habits and obeying traffic laws.
- Act as a member of the program team to review progress, plan future services, and communicate effectively with management staff, Service Coordinators, Clinicians, and other program staff.
- Complete and submit all documentation, including verification forms, checklists, progress notes, Habilitation Plans, and incident reports, by required due dates.
- Have the physical ability to move furniture when required and provide services to children and adults of all ages.
- Complete required trainings and attend meetings as requested by the Supervisor.
Requirements:
- A valid NYS Driver's license or the ability to convert an out-of-state license within 90 days.
- Ability to be certified in all required trainings.
- Occasionally required to lift up to 50 pounds.
- High School Diploma or GED required.
