Job Summary

We are seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a key member of our team, you will play a critical role in ensuring a smooth transition back home for our patients, while enjoying the one-on-one patient relationship unique to in-home care.

Requirements
  • Certified Nurse Practitioner or Physician Assistant
  • 1 year of primary care experience (preferred)
  • Ability to work independently and collaboratively as part of a team
  • Strong communication and interpersonal skills
