Position Overview:

As a vital member of our healthcare team, the Dietician - PRN will play a crucial role in providing nutritional support to patients in need.

Qualifications Required:
- Bachelor's Degree in Nutrition or a related field is preferred.
- A valid State Driver's License and reliable transportation are essential, along with current auto liability insurance.
- A minimum of two years of experience as a Dietician in a clinical environment, hospice, or home health setting is preferred.

- Proven ability to build and maintain effective working relationships with staff, patients, families, and medical professionals.

- A solid understanding of hospice care philosophy, focusing on comfort rather than rehabilitation, and the importance of maintaining care within the patient's residence.


Key Responsibilities:
- Conduct thorough dietary assessments for patients.
- Develop personalized care plans to address the dietary needs of each patient.
- Educate patients and caregivers on effective diet management strategies.
- Accurately document changes in patient condition and care provided to ensure seamless continuity of care.
- Offer guidance and education to the interdisciplinary group, nursing staff, and physicians as needed.
