Job Summary

We are seeking a skilled Speech Language Pathologist to join our team. As a Speech Language Pathologist at CPT Rehab, you will be responsible for providing home care and out-patient services to residents in the southwest suburbs, with a focus on geriatric rehabilitation. You will work closely with our interdisciplinary team to develop and implement individualized treatment plans that promote optimal patient outcomes.

Responsibilities
  • Provide home care and out-patient services to residents in the southwest suburbs
  • Develop and implement individualized treatment plans for patients with a focus on geriatric rehabilitation
  • Work closely with the interdisciplinary team to ensure comprehensive care
  • Document patient progress and outcomes using Kinnser and Thera Office documentation systems
Requirements

To be successful in this role, you will need to have a passion for geriatric rehabilitation and a strong commitment to delivering high-quality care. You will also need to have experience working with Kinnser and Thera Office documentation systems.
