We're seeking a Nurse Practitioner to join our team in Northern Colorado. Key Responsibilities:
* Deliver primary care at home visits to 8-10 patients daily
* Work a 4-day schedule, Thursday-Sunday
* Provide data-driven specialty visits as needed
* Manage patient outcomes and engage with population health data
* Develop strong relationships with care team members and external partners

Bloom Healthcare is an employee-owned and operated company committed to transforming healthcare delivery. We offer a competitive base salary of $90,000-$100,000 per year, plus a 10-15% annual quality incentive opportunity.
