Job Overview
As a Licensed Practical Nurse, you will be responsible for delivering high-quality patient care tailored to individual needs. Your role will involve assessing patients, documenting their medical histories, and monitoring vital signs to ensure optimal health outcomes.
Key Responsibilities
- Provide personalized, one-on-one care for clients, adhering to their specific care plans while respecting their dignity and independence.
- Monitor and report any abnormal vital statistics to the supervising case manager and physician.
- Administer medications, assist with feeding, and provide necessary care such as oxygen and ostomy management as outlined in the care plan.
- Collaborate with other healthcare professionals, including Physical, Occupational, and Speech Therapists, to complete established routines.
- Maintain strict adherence to infection control protocols, reporting any potential communicable diseases or infections.
- Document all nursing actions and communicate effectively with family members, case managers, and physicians regarding patient progress and changes in condition.
- Manage inventory by ordering supplies and medications to ensure availability at all times.
- Ensure that all patient needs are met in accordance with physician directives.
- Document and report incidents involving clients, ensuring timely and accurate completion of incident reports.
- Oversee the maintenance of medical equipment and report any malfunctions for repair.
- Perform additional duties as assigned.
Qualifications
- Completion of post-secondary education necessary for licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
- Understanding of individual service plans, client rights, and health requirements.
- Possession of a current, valid license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing.
- Certification in CPR.
- Strong interpersonal skills, including customer service, patience, empathy, and compassion.
- Comprehensive knowledge of patient care policies and regulations.
- Familiarity with the symptoms and signs of various medical conditions.
- Willingness to travel between facilities as required.
