The Surgical Case Manager will work collaboratively with the clinical team and medical providers to provide discharge planning coordination and intervention. This role aims to promote patient wellness, improve care outcomes, and optimize the utilization of health services for patients with complex health needs.
Key Responsibilities- Assessment: Conduct initial and ongoing care management assessments to determine the care coordination and discharge planning needs of patients.
- Planning: Develop a focused, anticipated discharge plan of care for high-risk patients with identified needs.
- Intervention/Evaluation: Collaborate with the clinical team and medical providers to initiate the anticipated discharge plan for patients.
- Performance Improvement: Stay up-to-date with changing organizational, emergency department, care management, and clinical trends, regulatory matters, and third-party payer requirements related to clinical care, discharge planning, and precertification or aftercare benefits.
- BSN preferred; ADN required
- 3-5 years of related experience; at least 2 years in CM with Surgery or ICU preferred
- Maryland Registered Nurse License
- Intent to achieve MD licensure if out of state
- Case Mgmt cert preferred
