About the Role

We are seeking a highly skilled and motivated Pharmacist to join our team at PharMerica. As a Pharmacist, you will play a critical role in providing exceptional patient care and medication management services to our clients.

Key Responsibilities
  • Evaluate physician medication orders for appropriateness, potential interactions, and route of administration.
  • Maintain patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
  • Ensure laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaborate with the healthcare team to provide clinical and technical support as needed.
  • Document all communication with physicians, nurses, and other members of the healthcare team in the medical record.
Requirements
  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • One year experience in long-term care.
  • Licenses/Certifications: As required by government regulations and business needs.
