About the Role: Mass General Brigham is seeking a skilled Physical Therapist to provide exceptional care services in Randolph, Massachusetts. As a Care Specialist, you will be working with patients in their place of residence, helping them achieve optimal independence and quality of life.

**Estimated Salary:** $63,500 - $82,500 per year, depending on experience and qualifications.

Job Summary: This position requires working every Saturday and Sunday, 12-hour shifts, serving patients in the North of Boston area. A generous sign-on bonus of $5,000 is offered to welcome new team members.

**Responsibilities:**
  • Develop personalized care plans that cater to patients' unique needs and goals.
  • Collaborate with patients, families, physicians, and other healthcare professionals to ensure seamless coordination of care services.
  • Apply standardized interventions to achieve patient-centered outcomes.
  • Provide education and support to patients and caregivers to empower them in managing their care.


Qualifications and Requirements: To excel in this role, you should have:
  • At least one year of experience as a Physical Therapist in acute care, rehab, or homecare settings.
  • Registration with the Board of Registration in Massachusetts.
  • Strong customer service skills and ability to work with diverse populations.
  • Excellent verbal, written, and electronic communication skills.
  • Proficiency in computer software applications.
  • Ability to adapt to changing situations and prioritize work effectively.
