Primary Care Physician

As a key member of our multi-disciplinary team at Southeast Primary Care Partners, you will play a vital role in delivering high-quality primary medical care to our patients. Your responsibilities will include providing diagnostic and therapeutic services, referring consultations for specialty care when necessary, and offering education and preventive care advice to all patients served.
Key Responsibilities- Develop strong, personal relationships with patients to ensure their unique needs are met.
- Assess, plan, and implement timely care for patients, taking into account their individual circumstances.
- Create personalized health programs based on patient historical data, which will be continuously monitored and adapted as needed.
- Schedule and administer follow-up examinations to track patient progress and provide ongoing support.
- Offer comprehensive wellness exams to promote overall health and well-being.
- Administer vaccinations and perform medical procedures as required.
- Recommending specialists for treatment outside your purview when appropriate.
- Ensure compliance with all relevant rules and regulations, including those set by the Georgia Composite Board.
- Educate patients on health management and disease prevention strategies.
- Support the development and implementation of evidence-based practice guidelines.
- Provide clear and effective communication to patients, ensuring they understand their care plans and any necessary next steps.
- Assume responsibility for day-to-day supervision of physician assistants/nurse practitioners.
- Participate in peer review, performance improvement, continuing education, risk management, and Clinical Indicator selection processes.
- Collaborate with other healthcare professionals to ensure continuity of patient care.
