Bilingual Triage Registered Nurse

Position Overview:

The Bilingual Triage Registered Nurse plays a vital role within the clinical team at CareBridge, focusing on patient outreach, triage, and health education through telehealth services. This position is essential for individuals receiving Home and Community Based Services (HCBS) through state Medicaid programs. The nurse collaborates closely with families, caregivers, and healthcare providers to ensure patients receive the appropriate care necessary for maintaining their health at home.

Key Responsibilities:

  • Deliver compassionate and culturally sensitive care to a diverse patient population.
  • Provide education and triage for patients facing a range of health concerns, from routine to urgent care.
  • Collaborate effectively as part of an interdisciplinary team to facilitate seamless care transitions.
  • Promote high-quality, cost-effective care for patients.
  • Conduct follow-ups to monitor and manage chronic health conditions.
  • Respond to unplanned, urgent, or emergency patient needs as they arise.
  • Adjust care plans and visit schedules based on individual patient requirements.
  • Proactively engage in Advance Care Planning and identify decision-makers for patient care.
  • Participate actively in quality improvement initiatives and customer service programs.
  • Review and reconcile patient medications accurately.
  • Recognize and respond promptly to emergency situations.
  • Maintain effective communication with the entire care team, including primary care providers and care coordinators.
  • Utilize the Teach Back method to ensure effective patient education.
  • Document all patient interactions in accordance with established standards.
  • Engage in clinical case discussions to enhance patient care.
  • Exhibit excellent punctuality and attendance throughout work hours.
  • Manage inbound call volumes, averaging 4-8 hours, requiring immediate triage.

Qualifications:

  • Possession of an active Compact Multi-state RN License.
  • Fluency in Spanish is required.
  • Experience in caring for adult patients with chronic illnesses.
  • Proficient in using electronic documentation systems.
  • Exceptional communication skills, particularly over the phone, to provide timely health advice.
