We are seeking a highly skilled Medical Health Specialist - Community Care to join our team at Partners For Quality As a key member of our team, you will play a critical role in coordinating physical health-related processes and ensuring that our clients receive the best possible care.

The successful candidate will have at least 5 years of experience in medical/surgical nursing, with a strong ability to communicate and establish relationships with others. As a Registered Nurse or LPN, you will be responsible for developing and implementing training programs to increase competency and capacity of medical healthcare in the community. You will also assist with reviewing health practices and policy development, analyzing aggregate health patterns and trends, and maintaining professional relationships with Agency Providers, staff, Supports Coordinators, and Case Managers.
