We are seeking a compassionate and skilled Medical Case Manager to join our team at Caracole Inc.

About the Role

This is an exciting opportunity for a dedicated professional to provide medical case management services to individuals and families living with HIV/AIDS. As a Medical Case Manager, you will be responsible for:

  • Providing medical case management services, including interviewing, assessment, service planning, referrals, follow-up, and evaluation.
  • Educating clients about safer sex practices, medication adherence, and linkage to medical care.
  • Participating in individual/group supervision meetings.
  • Maintaining accurate client and statistical records as required by social work practice, regulatory agencies, and Caracole Inc.
  • Meeting all required deadlines for documentation.
  • Maintaining a collaborative working relationship with Caracole team members.
  • Identifying and pursuing educational opportunities to promote professional growth.
  • Representing Caracole Inc in the community.
  • Providing transportation when needed.

Qualifications and Requirements

To be successful in this role, you will need:

  • A minimum of a Bachelor's Degree in Social Work (BSW) or Master's Degree in Social Work (MSW).
  • An active LSW license in Ohio or the ability to obtain it within 90 days.
  • Two years' experience as a social worker preferred.
  • Excellent skills in interviewing, assessment, service planning, service delivery, and program evaluation.
  • Knowledge about HIV/AIDS service networks, entitlement programs, and other community services.
  • Proven ability to work harmoniously with ethnically, culturally, and racially diverse groups of individuals.
