Under the general administration of the Director of Nursing and in collaboration with a physician and accountability to the Medical Director and/or designee, the Adult Health Nurse Practitioner will function in an expanded role in the area of ambulatory medicine.
Key Responsibilities:- Provides medical services within the scope of approved privileges.
- Documents all patient contact in the EMR in a timely manner.
- Assumes responsibility for their panel of patients and those patients they care for.
- Collaborates to provide care coordination of activities in providing comprehensive care.
- Provides relevant health education and instruction; counsels patient/family/responsible other on the impact of illness and principles of health promotion and maintenance.
- Provides clinical and didactic instruction to professional staff, peers, students and community groups and conducts in-service educational programs, as needed.
- Participates in departmental meetings and interdisciplinary or community committees, as well as Peer Review Performance Improvement meetings and activities, as required.
- Follows Gotham, NYC HHC and facility-specific policies and protocols.
- Meets all compliance standards and requirements.
- Ensures that credentials are maintained.
- Maintains Patient Safety and Infection Control Protocol as per Network and Departmental Policies.
- Upholds environmental safety standards to assure safety of patients and staff.
- Fosters/exercises Service Excellence in daily activities.
- A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
- Current certificate as a Nurse Practitioner issued by the NYSED.
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
- Satisfactory completion of an approved Nurse Practitioner graduate course consisting of classroom and clinical experience and two (2) years of satisfactory advanced clinical nursing experience.
- Certified and Licensed as Adult Health Nurse Practitioner by New York State Department of Education.
- Valid NYS License and current registration to practice as a Registered Professional Nurse.
