Job Title: Admission RN, Field RN | Home Health

ChristianaCare is seeking a highly skilled and dedicated Registered Nurse (RN) to join our Home Health team in Georgetown, Delaware.

About the Role:

We are looking for a compassionate and experienced RN to provide high-quality care to our patients in their homes. As an Admission RN, you will be responsible for performing clinical assessments, documenting patient records, and collaborating with interdisciplinary teams to ensure seamless care transitions.

Responsibilities:
  • Perform and document clinical assessments of patients at time of admission only
  • Precept other nurses who are taking on some admission duties
  • Initiates and participates in team meetings and/or independently collaborates in interdisciplinary discussions regarding patient care or practice
  • Enhances professional growth by staying current in clinical practice, incorporating research into practice, and participating in councils/committees/task forces
Requirements:
  • BSN required; ADN candidates are required to acquire their Bachelors of Science, Nursing Degree (BSN), within three (3) years of their start date as outlined in the Education Agreement required to be signed as a condition of employment
  • 2.5 years of Recent Skilled Adult Home Health experience required; must include OASIS documentation experience
  • OASIS proficiency required; acquire OASIS certification within 18 months of start date
  • IV proficient or willing to train, and achieve proficiency within 3 months of start date
