The Communication Specialist in Home Health is accountable for assessing and evaluating patient care requirements related to addressing speech and language challenges, as well as providing functional training in communication, swallowing, and cognitive difficulties.

  • Delivers services within the defined scope of practice as established by state regulations governing speech therapy, in alignment with the care plan, and in collaboration with other healthcare professionals.
  • Assesses the patient's functional level by utilizing diagnostic and prognostic functional ability assessments. Aids the physician in formulating the therapy care plan.
  • Facilitates effective communication for patients by helping them articulate their thoughts in accordance with their condition, adhering to accepted standards of practice.
  • Educates the patient, their family and/or caregiver, and other healthcare team members in relevant areas of speech therapy in which they can engage.
Licensure Requirements
  • Must hold a current license in Speech Therapy in the state of practice.
  • Current CPR certification is mandatory.
  • Must possess a valid driver's license and vehicle insurance, along with access to reliable transportation or public transit.
