About the Role

You will be responsible for conducting comprehensive assessments of patients' functional status, developing tailored treatment plans, and collaborating with other healthcare professionals to ensure seamless care delivery. Your expertise will enable clients to regain independence, improve mobility, and enhance overall well-being.

Duties and Responsibilities
  • Perform thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
  • Create and implement customized intervention plans to promote optimal outcomes.
  • Evaluate home environments to identify potential hazards or barriers to independent living.
  • Recommend necessary equipment and environmental adaptations to enhance client outcomes.
  • Maintain accurate documentation of patient care management and services provided, utilizing state-of-the-art technology.
Qualifications
  • Current MD Occupational Therapist license.
  • Prior home care experience advantageous, but not essential.
  • Minimum one year of recent work experience as an Occupational Therapist.
  • Accredited occupational therapy program graduation.
  • Excellent time management and problem-solving skills.
  • Proficiency in electronic medical records (EMR).
