Physical Therapist

We are seeking a highly skilled Physical Therapist to join our team at Tabitha Health Care Services. As a Physical Therapist, you will play a vital role in enhancing the living experience for our seniors by providing Physical Therapy services to maximize functional independence.
Key Responsibilities- Evaluate seniors utilizing appropriate diagnostic muscle, nerve, joint, and functional ability tests.
- Perform Physical Therapy treatment procedures or delegate to a licensed Physical Therapist Assistant based on initial and on-going senior evaluation.
- Develop and implement the senior's plan of care in collaboration with the physician, other consulting healthcare professionals, and the family/caregiver according to agency procedures and state and federal regulations.
- Document the care provided, instructions given, senior's progress toward goals, and changes in condition adhering to Tabitha's policies and procedures and state and federal regulations.
- Communicate and coordinate care and senior's progress with other team members involved in the care.
- Graduate of an accredited Physical Therapist school.
- One year experience as a Physical Therapist, preferred. Geriatric experience, preferred.
- Physical Therapist license in good standing as recorded on the Nebraska State Registry required.
- CPR Required.
- Valid Nebraska Driver's license (unless otherwise allowed by law) and a good driving record required.
