POSITION SUMMARY
The physical therapist engaged with Symbii Home Health and Hospice is accountable for delivering comprehensive physical therapy services while ensuring compliance with all stipulations outlined in the Personnel Services Agreement.
- Oversees the continuous interdisciplinary evaluation and formulation of the personalized care plan in collaboration with the patient, their representative (if applicable), and caregivers;
- Offers counseling to patients, caregivers, and family members;
- Educates patients and caregivers regarding their conditions and treatment;
- Documents clinical notes and progress reports based on goal achievement;
- Maintains communication with the physician overseeing the home health care plan and other healthcare professionals (as necessary) regarding the current home health care strategy;
- Delivers physical therapy services to patients in accordance with a physician's prescribed care plan.
- Assessing and evaluating therapeutic, rehabilitative, and functional status, while contributing to the overall care plan. Evaluates the home environment.
- Assessing muscle strength, mobility, gait, and range of motion with potential for rehabilitation.
- Directing physical therapy interventions.
- Instructing patients and families/caregivers on the use and maintenance of therapeutic devices.
- Identifying priority needs for physical therapy.
- Reporting to the physician regarding the patient's response to treatment or any changes in condition.
- Initiates the physical therapy program and instructs other staff and/or family members in specific aspects of physical therapy relevant to the patient, while also clarifying the goals of the therapy program during case discussions.
- May provide training to patients on the use of prosthetic devices.
- Identifies patient and family/caregiver needs for additional home health services and makes referrals as needed.
- Participates in discharge planning for patients.
- Conducts in-service educational sessions for nursing personnel as required.
- Engages in peer consultation processes.
- When therapy is the sole skilled service, instructs, supervises, and evaluates home health aide care biweekly.
- Supervises Physical Therapy Assistants in accordance with organizational policies and state regulations.
- Contributes to the quality assessment and performance improvement initiatives of the home health agency and participates in sponsored in-service training.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
- Must be licensed to practice as a physical therapist within the state, if applicable.
- Must have passed a state-approved examination for physical therapists and possess a degree from a baccalaureate or master's program in physical therapy accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) or its successor.
- Minimum of two years of relevant experience as a physical therapist; experience in community or home health settings is preferred.
- Exhibits strong verbal and written communication and organizational skills.
- Must possess and maintain current CPR certification.
- Ability to drive and/or have access to transportation for agency-related travel is essential. Must maintain a valid driver's license, automobile insurance, and have access to a vehicle.
