Position Summary:
The Nurse plays a pivotal role within the patient-centered medical home team. This position is accountable for overseeing personalized patient care, enhancing and restoring the health of patients through the nursing process, and collaborating effectively with healthcare providers and multidisciplinary team members. The Nurse also offers both physical and emotional support to patients.
Qualifications:
- Diploma in nursing with a minimum of 2 years of relevant experience.
- Current licensure from the South Carolina State Board of Nursing.
- Certification in CPR (Cardiopulmonary Resuscitation) or BLS (Basic Life Support) is mandatory.
- Exceptional customer service skills and strong organizational abilities are essential.
- At least 2 years of experience in a primary or specialty care medical setting.
- Capability to thrive in a high-pressure environment.

- Ability to engage with patients, families, clinical and non-clinical staff, and providers from diverse backgrounds while maintaining a non-judgmental approach.
- Excellent verbal and written communication skills are required.
- Proficiency in collaborating with others to enhance quality and address patient needs.
- Capacity to take initiative and manage multiple tasks concurrently.
- Familiarity with policies, procedures, regulations, and protocols.
- Knowledge of database systems (e.g., EMR systems).
- Bilingual in English and Spanish is preferred.
- Up-to-date medical knowledge, including pharmacology (medication names and usage), is required.
- Minimum of two years of experience in a physician's office.
- Strong interpersonal skills and attention to detail.
- Ability to multitask effectively.
- Compassionate approach towards patient care.
- Skill in administering various treatments and medications as prescribed.
- Adaptability to various computer software applications.
Supervisory Duties:
- This role does not include direct supervisory responsibilities.
Essential Responsibilities:
- Evaluate patient needs based on data collected through history, observation, physical examination, telephone triage, and diagnostic analysis.
- Provide direct patient care, including triage, assessment, procedures, treatment, patient education, and follow-up instructions.
- Assist patients and providers in securing necessary medicinal prior authorizations.
- Ensure patients comprehend the purpose of their visit.
- Deliver preventative care information tailored to the patient's age, gender, and health status.
- Maintain proficiency in practical nursing principles to deliver effective patient care and treatment.
- Understand examination, diagnostic, and treatment room procedures.
- Interpret, adapt, and apply relevant guidelines and procedures.
- Basic knowledge of Critical Care and Chronic Illness Management is required.
- Participate in ongoing training opportunities.
- Contribute to office efficiency and patient encounter volumes.
- Administer vaccines and immunizations to pediatric and adult patients as needed.
- Manage daily clinical workflows, including referral management, telephone encounters, lab results, medication refills, and compliance with sample medication logs.
- Willingness to learn about AIDS Service Organizations and Community Health Centers.
- Assist in onboarding new nurses to the organization by familiarizing them with standard operating procedures.
- Perform all other duties as assigned.
Physical Demands:
Hearing: Sufficient to perform job duties in person and over the phone. Speaking: Must communicate clearly with patients in person and via telephone. Vision: Adequate visual acuity to perform job duties, including reading printed materials and computer screens. Other: Requires occasional lifting and carrying of items weighing up to 10 pounds unassisted in daily activities and up to 30 pounds for specific tasks. Frequent bending, reaching, and repetitive hand movements (especially for typing and writing), as well as standing, walking, squatting, and sitting, are required throughout a regular work shift.
