We are seeking a highly skilled Healthcare Provider for High-Risk Patients to join our team at Sentara Health. This is a unique opportunity to make a meaningful difference in the lives of patients with complex needs.
Key Responsibilities- Provide comprehensive care to patients with high-risk conditions, including geriatric medicine.
- Collaborate with hospitalists, specialists, and other healthcare providers to ensure seamless transitions of care.
- Develop and implement personalized care plans, promoting adherence to treatment.
- A competitive salary of $140,000 - $170,000 per year, depending on experience.
- A generous time-off policy, including holidays and vacation days.
- $3,000 CME allowance plus 5 CME days.
- $5,000 QPI annual bonus.
- $10,000 student loan forgiveness program.
