The Speech Language Pathologist in Home Health is tasked with assessing and evaluating patient care needs concerning speech and language disorders, as well as providing functional training in communication, swallowing, and cognitive challenges.

Responsibilities include:

  • Delivering services within the legal scope of practice as defined by state regulations.
  • Collaborating with the healthcare team to ensure comprehensive patient care.
  • Evaluating patients' functional levels through diagnostic assessments.
  • Assisting physicians in formulating therapy plans.

Additionally, you will work with patients to enhance their communication abilities, guiding them to express their thoughts effectively based on their individual conditions. You will also instruct patients, families, caregivers, and other healthcare team members on how to engage in the speech therapy process.

Requirements:
- Current licensure in Speech Therapy in the state.
- Valid CPR certification.
- A current driver's license and vehicle insurance, along with access to reliable transportation.
