Our home health and hospice agencies are located throughout California, Hawaii, Oregon, and Washington, serving the entire community with personalized in-home care.
Key Responsibilities:- Assess and Document Patient Data: Conduct thorough assessments and document speech therapy data related to patient care needs, interventions, and outcomes.
- Identify Changes and Trends: Monitor physical and emotional parameters to identify changes and trends in patient progress.
- Review and Modify Treatment Plans: Regularly review treatment plans and assess individual performances to modify, change, or write new programs as needed.
- Maintain Accurate Documentation: Ensure compliance with legal, policy, and regulatory requirements by maintaining accurate and up-to-date documentation.
- Provide Patient Education: Instruct patients on ways to monitor speech and practice new skills to achieve optimal outcomes.
- Education and Work Experience: Bachelor's Degree or equivalent required; Master's Degree preferred.
- Licenses and Certifications: Speech Pathologist licensure in the state of practice required; Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required; American Speech-Language and Hearing Association (ASHA) Certificate of Clinical Competence (CCC) required.
