Home Health Registered Nurse

At Interim HealthCare, we value our nurses and empower them to be patient advocates. As a Home Health Registered Nurse, you will provide care to patients in their homes, often after discharge from a hospital, rehab, or nursing facility.

Responsibilities:
  • Examine patients, conduct assessments, and document progress
  • Work with a team of physicians and oversee LPNs, CNAs, and aides providing patient care
  • Educate patients on their plan of care, medication, therapy, diet, and exercise
  • Coach family members on the patient's plan of care, medication, and home safety
  • Ensure goals are met and coordinate discharge from services
Requirements:
  • Active Registered Nurse (RN) License in Colorado
  • Home health care experience helpful but not required
  • No Case Manager responsibilities
  • Graduate of an accredited school of nursing
  • At least 1200 hours of experience as an RN
  • Health care CPR/BLS Certified (demonstration course required)
  • Pass a background check and drug test (we are required to test for THC)
  • No Covid Vaccination Required
  • Must be willing to travel to Greeley, Fort Collins, Loveland, and surrounding areas
