Job Summary:
This role directs and provides nutrition care to patients through assessment, planning, implementation, and monitoring. The successful candidate will provide education to patients and staff, and perform documentation and reporting according to established procedures.
Key Responsibilities:
- Conduct individualized patient assessments and reassessments to develop and implement personalized nutrition plans.
- Develop, document, and monitor the nutritional plan of care for the population served, adhering to professional standards and guidelines.
- Provide education and/or educational programs for patients and staff to promote healthy nutrition practices.
- Perform documentation and complete reports according to established procedures and guidelines.
- Develop, implement, and monitor tools necessary to maintain and improve nutrition care provided to patients, staff, and community.
Qualifications:
- Bachelor's Degree in Dietetics or Nutrition.
- Registered Dietician Nutritionist (RDN) or Registered Dietician (RD) certification, or eligibility to obtain within six months of hire.
- Licensed Dietitian/Nutritionist (LDN) or eligibility to obtain within nine months of hire.
Preferred Qualifications:
- Master's Degree in Dietetics or Nutrition.
- Experience in specialty practice.
