This is a unique opportunity to join Vaya Health as an Embedded Care Manager, working closely with local DSS agencies to provide proactive intervention and coordination of care to eligible members. As a key member of the Care Management Team, you will be responsible for ensuring that members receive appropriate assessment and services, and that their needs are met through a comprehensive care plan.
The Embedded Care Manager will work with members, care teams, and other stakeholders to identify and alleviate inappropriate levels of care or care gaps. This will involve conducting assessments, developing care plans, and coordinating services across multiple health domains. You will also provide consultation, education, and system navigation to support DSS social workers and other care team members.
This is a mobile position that requires the ability to work in a variety of locations, including members' home communities. You will need to be able to communicate effectively with members, care teams, and other stakeholders, and to navigate complex systems and policies.
The ideal candidate will have a strong understanding of behavioral health, substance use, and long-term services and supports, as well as experience working with vulnerable populations. You will need to be able to drive and sit for extended periods of time, and to work in a fast-paced environment with multiple demands and priorities.
Key Responsibilities:
- Conduct assessments and develop care plans for members
- Coordinate services across multiple health domains
- Provide consultation, education, and system navigation to support DSS social workers and other care team members
- Work with members, care teams, and other stakeholders to identify and alleviate inappropriate levels of care or care gaps
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements:
- Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area
- Two years of experience working directly with individuals with behavioral health conditions
- Two years of prior long-term services and supports (LTSS) and/or home community-based services (HCBS) coordination, care delivery monitoring, and care management experience
- Ability to drive and sit for extended periods of time
- Ability to work in a fast-paced environment with multiple demands and priorities
Preferred Qualifications:
- Experience working directly with individuals with intellectual and developmental disabilities (I/DD) or traumatic brain injury (TBI)
- Knowledge of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5)
- Experience with care management software and electronic health records
