Job Overview

Nurse Practitioners play a vital role in our team, conducting comprehensive assessments, medical history reviews, diagnoses, treatment plans, health education, and referrals as needed. They collaborate with physicians, case managers, social workers, family members, and other healthcare professionals to ensure seamless patient care.

Key Responsibilities
  • Conduct Adult/Geriatric Assessments, reviewing medical history, diagnosis, and treatment plans while providing health education and referrals when necessary.
  • Work collaboratively with physicians, case managers, social workers, family members, and ancillary medical personnel to deliver comprehensive care.
  • Collaborate with Primary Care Physicians (PCPs) on patient education, provide follow-up support, and ensure continuity of care.
  • Provide services in various settings, including home visits, skilled facility visits, and telehealth sessions.
Requirements and Qualifications

To be successful in this role, candidates must have:

  • A Master's Degree or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification from AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • A current BLS, ACLS, or CPR certification.
  • A minimum of 1 year of experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with Employee Health/Medical Record software.
  • Excellent verbal and written communication skills for patient, client, and colleague interactions.
  • The ability to travel, possessing a valid state driver's license, and adequate automobile insurance coverage.
Compensation and Benefits

This part-time position offers $105 per completed assessment, flexible scheduling, mileage reimbursement, and opportunities for professional growth. As a PRN provider, you will receive benefits such as 401(k) and limited Voluntary Benefits.
