About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Adoration Home Health and Hospice. As a LPN, you will play a vital role in providing high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Assist with admissions, discharges, and follow-up care as needed.
  • Participate in medical appointments and provide support as required.
  • Deliver nursing services under the direction of the RN and as ordered by the physician.
  • Develop and implement individualized care plans to meet patients' unique needs.
  • Monitor and record vital signs, weights, wound measurements, and other essential health metrics.
  • Identify trends and concerns, and communicate with RNs and physicians to ensure timely interventions.
Requirements
  • Current licensure as a Licensed Practical/Vocational Nurse (LPN or LVN) in good standing.
  • CPR certification.
  • At least six months of direct nursing experience.
