Job Summary:

We are seeking a highly motivated and compassionate Care Advocate to join our team at Heartland Hospice. As a RN Clinical Liaison, you will play a vital role in educating healthcare professionals about hospice services and providing end-of-life solutions with clinical excellence, compassion, and dignity.

Your responsibilities will include ensuring the intake and referral processes are performed effectively and timely, completing admissions processes, and providing accurate and timely information regarding hospice services.

With a strong background in healthcare and a passion for delivering exceptional patient care, you will be an integral part of our team and make a lasting difference in the lives of our patients and their families.

About You:
  • You will report directly to the AVP of Business Development.
  • You have a minimum of 2 years of experience in one of the following settings: hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, or hospice case manager.
