Position Overview: Licensed Practical Nurse - The Licensed Practical Nurse (LPN) is responsible for comprehensive resident care within a designated unit during each shift. This role encompasses a variety of professional nursing responsibilities while ensuring that nurse aide tasks are completed effectively. Collaboration with all healthcare disciplines is essential to maintain high standards of resident care.
Key Responsibilities:
- Oversee and guide the nursing team on the floor in collaboration with the Director of Resident Services (DRS) to ensure optimal resident care.
- Develop nurse aide assignments as necessary to address resident needs.
- Alert the DRS regarding any changes in resident conditions or staff-related issues.
- Administer medications and treatments following established protocols and guidelines.
- Ensure the medicine cart is secured when not in direct view.
- Regularly update resident support plans as required.
- Conduct thorough assessments of residents and adhere to established care policies.
- Assist residents with Activities of Daily Living (ADLs) as needed.
- Support residents during meal times to promote proper nutrition and hydration.
- Accompany physicians during resident examinations to ensure continuity of care.
- Timely transcription of physician orders in accordance with policy.
- Monitor nursing care and engage with residents throughout the shift to ensure adherence to care plans.
- Participate in shift report exchanges to guarantee continuity of care and relay essential information to nurse aides.
- Supervise the care provided by Nurse Aides.
- Complete incident and accident reports in accordance with policy promptly.
- Collect necessary samples, such as urine or sputum, for testing when required.
- Communicate vital information with families, residents, and staff as necessary.
- Ensure that adequate supplies are available on the floor for all staff members.
- Engage in staff meetings to enhance communication across all shifts.
- Contribute to resolving employee and floor-related issues.
- Participate in the orientation and training of new personnel.
- Maintain a clean, safe, and organized environment for residents, staff, and visitors.
- Document all care provided to residents accurately and in a timely manner.
- Chart medications immediately following administration, in compliance with policy.
- Provide feedback to the DRS regarding nurse aide performance.
- Uphold confidentiality at all times.
- Attend and engage in staff meetings, mandatory training sessions, and continuing education opportunities.
- Always operate within the guidelines of the Pennsylvania State Professional Standards Title 49 Chapter and adhere to the scope of practice permitted by licensure.
- Perform additional duties as assigned.
Qualifications:
- Graduation from an accredited nursing program and possession of a current, valid Practical Nurse license in good standing from the Commonwealth of Pennsylvania. A temporary practice permit is required for nurses awaiting licensure.
- Proven management, communication, and observational skills with a focus on geriatric care.
- Proficiency in computer usage, particularly Microsoft Word.
