Job Summary

The Hospice Social Worker is a vital member of the Compassus team, responsible for providing psychosocial support and counseling services to patients and their families. This role requires a strong understanding of the Compassus values and philosophy, as well as the ability to work effectively with the interdisciplinary team (IDT) to develop and implement comprehensive care plans.

Key Responsibilities
  • Explain hospice services and Medicare benefits to patients and families, obtaining informed consent and election of benefits documents as needed.
  • Submit accurate and timely documentation and paperwork to facilities, adhering to company policy.
  • Participate in IDT meetings, contributing to the development and implementation of patient care plans.
  • Provide psycho-social support and counseling services to patients and families, collaborating with external resources as needed.
  • Identify and address patient and family needs, communicating effectively with the IDT to ensure continuity and quality of care.
  • Maintain comprehensive care plans, reflecting current problems, goals, and interventions for patients.
  • Respect patient and family environments and beliefs, maintaining a non-discriminatory approach.
  • Complete initial and ongoing psycho-social assessments, addressing concerns related to financial means, insurance, living arrangements, caregiver stressors, and community resource needs.
  • Facilitate placement in long-term care as needed, monitoring adjustment and participating in care planning meetings.
  • Support advance planning, including living wills, POA documents, funeral arrangements, and memorial services.
Requirements
  • Minimum of Bachelor's degree in Social Work, Psychology, Sociology, or related field, with one year of social work experience in a healthcare setting.
  • Master's degree in Social Work with one year of experience in a healthcare setting highly preferred.
  • Licensure required by state rules, with supervisory oversight by MSW for hospice patient care planning and counseling needs.
Skills
  • Strong written and verbal communication skills, with ability to articulate and embrace the Compassus philosophy.
  • Ability to understand, read, write, and speak English.
  • Mathematical skills, including ability to add, subtract, multiply, and divide in all units of measure.
Work Environment
The demands of this role require a team member to effectively perform essential functions, with adaptations made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
