Hospice LPN Job Description

We are seeking an experienced Licensed Practical Nurse (LPN) to join our Hospice division at Ozark Total Healthcare. As a compassionate and dedicated healthcare professional, you will play a vital role in providing high-quality patient care and support to individuals and their families during their end-of-life journey.

Key Responsibilities:
  • Assist the Registered Nurse in carrying out the plan of care, ensuring seamless coordination and communication with the healthcare team.
  • Participate in the preparation, implementation, and ongoing evaluation of patient care plans, working collaboratively with the Interdisciplinary Group (IDG) to address any identified issues.
  • Reinforce teaching to patients and their families regarding end-of-life care, promoting understanding and empowerment.
  • Foster a cooperative environment among personnel, actively participating in team and staff conferences to ensure effective communication and collaboration.
  • Assume responsibility for personal and professional growth and development, engaging in ongoing education and training to enhance nursing practice.
  • Prepare accurate and timely clinical and progress notes, keeping the Case Manager informed of changes noted during visits.
  • Perform on-call duties, including holidays, as per policy and agency need.
Requirements:
  • Graduation from an accredited School of Practical Nursing.
  • Current licensure as a Licensed Practical Nurse in the State of Missouri.
  • One or more years of nursing experience, with Hospice experience preferred.
  • A valid driver's license with an insured automobile in good working order.
  • Excellent communication and interpersonal skills, both verbal and written.
