Position Overview:


Pomona Vista Care Center is seeking a dedicated full-time Rehabilitation Specialist. The compensation range is $48-$55 per hour, based on experience and qualifications.

Key Responsibilities:


The Rehabilitation Specialist will be responsible for delivering, overseeing, and managing all facets of high-quality patient care, which includes but is not limited to: assessment, evaluation, treatment, care planning, goal establishment, family education, and thorough documentation to ensure optimal patient outcomes.


  • Supervise and guide the activities of therapy assistants.
  • Formulate treatment plans and deliver services in alignment with reimbursement standards.
  • Maintain accurate records, including notes, summaries, and reports.

Qualifications:

  • Bachelor's degree in Physical Therapy.
  • State licensure as required.
  • Current CPR certification.
