Job SummaryThis is an excellent opportunity to work with a dynamic and growing team in a Skilled Nursing Facility (SNF) setting. As an Occupational Therapist, you will be responsible for evaluating and treating patients following an individualized plan of care.
Main Responsibilities- Patient Care: Provide high-quality care to patients, addressing their physical, emotional, and social needs.
- Customer Service: Deliver exceptional customer service to patients and their families, ensuring they feel valued and supported throughout their treatment journey.
- Collaboration: Work closely with facility staff to ensure seamless care coordination and outstanding patient outcomes.
- Assessment and Treatment: Conduct thorough assessments, develop personalized plans of care, and set achievable goals for each patient.
- Patient Education: Educate patients on their condition, progress, and potential outcomes, empowering them to take control of their recovery.
- Progress Monitoring: Regularly review patient progress, revise plans of care as needed, and reset goals to ensure optimal outcomes.
- Documentation: Maintain accurate and timely documentation, including evaluations, re-evaluations, discharges, and daily progress notes.
- SUPERVISION: Supervise Occupational Therapy Assistants, ensuring they provide high-quality support to patients and meet established standards.
