We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Healthcare LPN - Patient Care Specialist.

Job Responsibilities:

  • Patient Care: Perform patient assessments, develop individualized care plans, administer medications, treatments, and therapies under physician orders.
  • Monitoring and Reporting: Continuously observe and assess patient condition, report changes to the supervising RN or physician as necessary.
  • Care Coordination: Collaborate with the interdisciplinary team to ensure coordinated and comprehensive care delivery.
  • Documentation: Maintain accurate and up-to-date patient documentation in accordance with regulatory standards and organizational policies.
  • Education and Support: Educate patients and families on healthcare management, medication administration, and self-care techniques.

Requirements:

  • Licensure: Current and unrestricted LPN license in Pennsylvania.
  • Experience: Minimum six months of clinical experience, preferably in home healthcare or pediatric settings.
  • Skills: Strong clinical assessment and critical thinking skills, excellent communication and interpersonal skills.
  • Physical Demands: Ability to lift 50 pounds, prolonged walking, standing, bending, kneeling, reaching, twisting, and sitting.
