We are seeking a compassionate and skilled Home Health Licensed Practical Nurse to join our team at Interim HealthCare-Ringling Group. As a Home Health LPN, you will have the opportunity to provide high-quality patient care in a home-based setting.

Job Summary

The successful candidate will be responsible for providing direct patient care, working collaboratively with a multidisciplinary team, and educating patients and their families on self-care techniques and medication management.

Key Responsibilities
  • Provide home-based care to patients with various medical conditions, including illnesses, injuries, and chronic diseases
  • Work closely with physicians, RNs, CNAs, and aides to deliver coordinated patient care
  • Take vital signs, conduct assessments, document progress, and report changes to the RN supervisor
  • Assist RNs in implementing patient care plans and teaching self-care techniques
  • Educate patients and their families on medication management, home safety, and disease prevention
Requirements
  • Graduate of an accredited practical nursing program and active LPN license in Colorado
  • Minimum six (6) months of LPN experience, preferably in home healthcare or completion of office's preceptor program
  • CPR Certification required
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing, strong communication skills, and compassion
