At Crossroads Hospice & Palliative Care, we are seeking a compassionate and skilled Clinical Liaison Registered Nurse to join our team. As a Clinical Liaison RN, you will play a vital role in delivering exceptional patient care and support to our patients and their families.
Key Responsibilities:
- Provide symptom relief and support to patients and their families
- Collaborate with healthcare teams to ensure seamless care transitions
- Develop and maintain strong relationships with patients, families, and healthcare providers
- Participate in marketing efforts to promote our services and build community partnerships
Qualifications:
- 2 years of prior skilled Registered Nursing experience
- Active unencumbered state RN license
- Current driver's license and auto liability coverage
- CPR certification, a copy of TB Results, a copy of an updated physical
- Strong understanding of Hospice and Palliative Care
- Strong understanding of a hospital environment
- Interest in Marketing
- Strong relationships within the Philadelphia community
