Key Responsibilities:
- Lead an interdisciplinary team in providing care to seniors in various settings, including PACE centers.
- Provide consultative geriatric services to primary care physicians, identifying patients ready for transition to post-acute services.
- Collaborate with hospitalists, primary care physicians, and other healthcare professionals to develop comprehensive care plans.
