Optum Case Manager RN Job Description

At Optum, we're dedicated to delivering care that improves health outcomes and enhances the patient experience. As a Case Manager RN, you'll play a critical role in coordinating patient services through an interdisciplinary process, ensuring timely care delivery and optimal resource utilization.

Key Responsibilities:
  • Assess patient needs and develop individualized care plans to achieve desired quality outcomes.
  • Collaborate with healthcare teams to facilitate effective communication and ensure seamless transitions of care.
  • Monitor and report quality and risk management issues to ensure compliance with CMS guidelines and regulatory requirements.
  • Develop and implement discharge plans that promote continuity of care and optimize patient outcomes.
  • Provide education and support to patients and families to empower informed decision-making and self-management.
Requirements:
  • Associate's Degree in Nursing (or higher) and current, unrestricted RN license in the state of employment.
  • 1+ years of experience in case management, clinical care coordination, or a related field.
  • Strong analytical, critical thinking, and organizational skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
  • Knowledge of utilization review, Medicare requirements, and state and federal regulations related to post-acute services.
