The Registered Nurse Salaried is responsible for delivering high-quality, patient-centered care in the comfort of patients' homes. This role requires a strong understanding of skilled nursing principles and the ability to work independently with minimal supervision.
Key Responsibilities- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Provide education and support to patients and their caregivers on disease management, medication adherence, and self-care techniques.
- Conduct thorough assessments, identify areas of concern, and develop strategies to address them.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Document all patient interactions, assessments, and interventions in a timely and accurate manner.
- Active RN license in the state of application.
- Valid CPR certification.
- 1 year of experience in a healthcare setting, preferably in skilled nursing or home health.
- Basic understanding of OASIS and other relevant healthcare software.
