The Resource Nurse will work closely with the Emergency Department (ED) and Regional Care Coordination (RCC) teams to provide support and assistance in various communities. This role will involve collaborating with healthcare professionals to ensure the smooth completion of tasks, such as quarterly pharmacy reviews, Risk Watch, admissions/re-admissions, lab results, and physician visits.
Key Responsibilities- Community Support: Provide assistance to ED and RCC teams in communities, ensuring timely completion of tasks and maintaining facility compliance.
- Compliance and Quality Assurance: Monitor and assist with compliance with policies, procedures, and state and federal regulations, including blood-borne pathogens, infection control, and resident rights.
- Admission and Assessment: Assist residents with admission, ensuring compliance with all state and federal requirements, including initial Advance Directives, Resident Rights information, and assessments.
- Communication and Collaboration: Maintain open communication with families, physicians, and pharmacy, as required, to ensure seamless care coordination.
- Float and Support: Expected to float to each community on a scheduled rotation, unless assigned to a specific community for a specified time, to provide support and assistance as needed.
- Audit Compliance: Assist communities with audit compliance, ensuring timely completion of tasks and maintaining facility compliance.
- Travel and Mobility: Travel to communities is required, and the ability to work beyond normal working hours and on weekends and holidays when necessary.
- Professional License: Must have a valid professional license (RN or LPN) and be in good standing with the state.
- Computer Skills: Must have basic computer skills and be able to operate various software systems.
- Communication and Interpersonal Skills: Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the community.
- Physical Demands: Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
