Job Description:

As a Physical Therapist with Intermountain Health, you will play a vital role in providing patient-centered care in the comfort of patients' homes. Our team is committed to delivering high-quality, compassionate care that meets the unique needs of each patient.

Key Responsibilities:
  • Conduct thorough assessments of patients' conditions and develop personalized treatment plans to improve mobility, relieve pain, and prevent disability.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Utilize a range of modalities, including exercise regimens, adaptive equipment, and support programs, to promote patients' independence and well-being.
  • Document patient progress and outcomes in accordance with home health and regulatory guidelines.
Requirements:
  • Current/valid state Physical Therapist licensure in the state of practice.
  • Bachelor's Degree in Physical Therapy from an accredited school.
  • Basic Life Support (BLS) Certification for healthcare providers endorsed by the American Heart Association.
  • Current driver's license, insured, and reliable transportation, and an acceptable driving record.
  • Ability to communicate effectively both verbally and in writing.
Preferred Qualifications:
  • Previous experience in Home Health, Acute Care, or Clinic setting.
  • Doctorate Degree in Physical Therapy, preferred.
Physical Requirements:
  • Carrying, Climbing, Crawling, Hearing/Listening, Lifting, Manual Dexterity, Pulling/Pushing, Seeing, Speaking, Squatting/Kneeling, Standing, Walking.
